| APPLICATION FOR INTRASTATE MOTOR CARRIER AUTHORITY |
| All household goods and passenger carriers operating for-hire point to point within Nebraska must obtain intrastate authority. |
- Carrier must complete the intrastate application form and return it with the $300.00 filing fee.
- The application must contain the name, business address, and telephone number of the motor carrier applying for authority.
- Carrier must check if it is an individual, partnership, corporation or association.
- Carrier must complete either Part I or Part II of the application, whichever is applicable.

| Part I |
| Carrier must complete Part I if seeking new or extending authority. Carrier needs to indicate if instituting a new operation or extending its authority and whether carrier is seeking to perform intrastate transportation as a common carrier or a contract carrier (if as a contract carrier, must indicate for who). Carrier must also indicate whether it will be transporting passengers or property. If transporting passengers, must indicate if by taxi, limousine, van, bus, or other. Carrier must indicate if seeking to transport over regular or irregular routes and describe in detail the area of Nebraska operations which will be conducted. |
| Part II |
| Carrier must complete Part II if seeking to change an existing authority. Carrier needs to indicate if seeking to change an existing operation by sale or transfer of stock ownership, change in partnership, consolidation, merger, transfer, lease, or other. Carrier must provide the name of the holder of authority and certificate/permit number for which a change is proposed. |
- Carrier must complete Part III by indicating what is attached to the application: $75.00 application fee (required), copy of contract, (required if seeking contract carrier authority), copy of Articles of Incorporation, required if a corporation), copy of Articles of Partnership, (required if a partnership), and purchase agreement (required if a transfer of authority).
- Carriers applying for new or extended authority must complete the support information, also. Carrier must list the name, contact person, address, and phone number of two individuals, corporations, associations or partnerships that agree to support the application.
- Carrier must complete the financial statement section by listing assets and liabilities.
- Carrier must sign and date application and provide name, address, and phone number of attorney representing them (if applicable)

| After application is received, it is published in the Daily Record, Omaha, Nebraska, and there is a 30-day protest period after that. |
